When an internal organ or other body component protrudes through the muscle or tissue wall that typically contains it, a hernia is said to have occurred. Majority of hernias occur between the chest and the hips, in the abdominal cavity. Some common forms of hernia are given below.
Inguinal hernia: This type of hernia affects men more as compared to women. An inguinal hernia refers to a condition where fatty tissue or a part of the intestine pushes into the groin at the top of the inner thigh.
Femoral hernia: Femoral hernia is less common than inguinal hernia, and mostly affects elderly women. While femoral hernias are a type of groin hernia, these take place lower in the body as compared to inguinal hernia. Femoral hernias occur in the upper part of the thigh close to the groin just below the inguinal ligament, where abdominal contents pass through the femoral canal.
Umbilical hernia: In this type of hernia, fatty tissue or part of the intestine bulges through the abdomen near the navel.
Hiatal (hiatus) hernia: This type of hernia takes place when a part of the stomach pushes up into the chest cavity through an opening in the diaphragm.
Incisional hernia: In this type of hernia, intestine, organ or tissue thrusts through or near the site of an abdominal scar from a remote abdominal or pelvic surgery.
Epigastric hernia: Fatty tissue bulges through the abdominal area between the navel and the lower part of the sternum.
Spigelian hernia: This type of hernia takes place when the intestine juts through the spigelian fascia or layer of tissue that separates two groups of abdominal muscles.
Together, inguinal and femoral account for a majority of all hernia cases.
A hernia may require surgery to be treated. Normally, the following surgical procedures are performed:
Open surgery – The bulging tissue is pushed back in place through a cut in the body at the hernia site, and the weakened muscle wall stitched back together. A mesh may be implanted in the area to offer extra support. While mesh prosthetics have been generally employed in most countries for the treatment of hernia, associated problems such as seroma formation, mesh shrinkage or encapsulation, tissue degradation and chronic pain have been reported. Currently, the most common treatment alternatives involve the use of prosthetic biomaterials (absorbable, non-absorbable, composite, coated, impregnated), xenografts and allografts.
Laparoscopic surgery – The same type of repairs is performed, however, instead of a large cut, small incisions are made to insert surgical tools to carry out the procedure. The laparoscopic approach causes less pain, and enables earlier return to normal activities and work as compared to the open approach. However, it requires administration of general anesthesia and is costlier than the open approach.
Robotic hernia repair – In this technique, the surgeon sits at a console and controls the surgical instruments from the console. Robotic surgery offers advantages such as three-dimensional images of the inside of the abdomen, smaller scars, and less pain.
