The global extracorporeal carbon dioxide removal (ECCO2R) devices market is expected to grow at a CAGR of 6.90% during the period 2025-2034. Significant technical advancements have improved extracorporeal techniques for carbon dioxide removal; such changes may help avoid worsening respiratory failure and respiratory acidosis, and possibly prevent or reduce the duration of IMV in patients with aggravation of COPD and asthma. North America, Europe and Asia are expected to be key markets.

Extracorporeal Carbon Dioxide Removal (ECCO2R) Devices Market Outlook
*this image is indicative*
Respiratory failure is a condition often faced in the practice of critical care. In cases where there is an ineffective exchange of gases (such as in severe respiratory insufficiency), life-saving measures including mechanical ventilation are considered. While with significant benefit, positive-pressure ventilation has a risk of leading to complications like oxygen toxicity, lung-hyperinflation, and ventilator-associated pneumonia, among others. In such conditions that warrant gaseous exchange despite applicable MV settings, extracorporeal membrane oxygenation (ECMO) devices are employed. ECMO devices are available in different forms according to the circuit configured and the elements forming it.
Configurations that involve draining deoxygenated blood from the venous compartment and directing oxygenated-decarboxylated blood to a vein is called venovenous ECMO (VV-ECMO). When the blood is configured to return to the artery, it is veno-arterial ECMO (VA-ECMO). Extracorporeal carbon dioxide removal (ECCO2R) devices refer to specialized ECMO devices that primarily focus on CO2 removal, thereby decreasing PaCO2 and, in due course, the work of breathing and MV support. The potential advantage of extracorporeal carbon dioxide removal (ECCO2R) devices is decreased blood flow through the circuit. The membrane in action comprises polymethylpentene (PMP) or siloxane.
Arterio-venous extracorporeal carbon dioxide removal circuits do not require the pump in the configuration for functioning, while venovenous extracorporeal carbon dioxide removal requires pump.
Over the last decade, extracorporeal support for respiratory failure has garnered much interest, in particular, extracorporeal carbon dioxide removal (ECCO2R), owing to relative ease and efficiency in blood CO2 clearance afforded by extracorporeal gas exchangers against oxygen delivery. Recent years have witnessed a new generation of extracorporeal carbon dioxide removal (ECCO2R) devices. Such developments are expected to drive the global extracorporeal carbon dioxide removal (ECCO2R) devices market. Increasingly efficient venovenous (VV)-ECCO2R devices have become available and replaced the arterio-venous approach, with the advantage of not needing arterial puncture. Modern VV-ECCO2R devices provide lower resistance to blood flow, possess smaller priming volumes, and offer a more efficient gas exchange with comparatively low extracorporeal blood flows (0.4–1 L/min).
Companies are engaged in providing modern solutions. For example, ApherCap is a simple multi-therapeutic platform that enables extracorporeal treatment for respiratory failure in acute critically ill patients, particularly: CO2 removal (ECCO2R); CO2 removal in combination with hemofiltration (ECCO2R + HF); and Emoperfusione (HP). In ECCO2R mode, ApherCap administers extra-corporeal CO2 removal therapies, decreasing the patient’s ventilatory needs with respiratory failure. Such solutions are likely to boost the global extracorporeal carbon dioxide removal (ECCO2R) devices market. The use of this method has been proposed in several clinical scenarios.
Extracorporeal carbon dioxide removal (ECCO2R) enables protracted treatments up to five days without replacement/modification of disposable kit; enables variable blood flow between 30 and 450 ml/min; and has a user-friendly interface.
