Respiratory failure is a condition often faced in the practice of critical care. In cases where there is an ineffective exchange of gases (such as in severe respiratory insufficiency), life-saving measures including mechanical ventilation are considered. While with significant benefit, positive-pressure ventilation has a risk of leading to complications like oxygen toxicity, lung-hyperinflation, and ventilator-associated pneumonia, among others. In such conditions that warrant gaseous exchange despite applicable MV settings, extracorporeal membrane oxygenation (ECMO) devices are employed. ECMO devices are available in different forms according to the circuit configured and the elements forming it.
Configurations that involve draining deoxygenated blood from the venous compartment and directing oxygenated-decarboxylated blood to a vein is called venovenous ECMO (VV-ECMO). When the blood is configured to return to the artery, it is veno-arterial ECMO (VA-ECMO). Extracorporeal carbon dioxide removal (ECCO2R) devices refer to specialized ECMO devices that primarily focus on CO2 removal, thereby decreasing PaCO2 and, in due course, the work of breathing and MV support. The potential advantage of extracorporeal carbon dioxide removal (ECCO2R) devices is decreased blood flow through the circuit. The membrane in action comprises polymethylpentene (PMP) or siloxane.
Arterio-venous extracorporeal carbon dioxide removal circuits do not require the pump in the configuration for functioning, while venovenous extracorporeal carbon dioxide removal requires pump.
