Adrenaline, also known as epinephrine, is a hormone and medication produced by the adrenal glands that helps regulate visceral functions like respiration. It is released during times of stress or danger, preparing the body for a "fight or flight" response by increasing heart rate, blood pressure, and breathing. Adrenaline also plays a role in emotional responses, memory consolidation, and treating allergic reactions, cardiac arrest, and asthma. The first purification of adrenaline was achieved by Japanese scientist Jokichi Takamine in 1900. In 1901, Takamine and Parke-Davis both obtained patents for adrenaline.
Adrenaline is a white, microcrystalline powder or granule that is odourless and has a slightly bitter, numbing taste. It has a melting point of 211-212°C and aqueous solutions are slightly alkaline. Adrenaline is highly toxic if ingested, inhaled, or absorbed through the skin, and can be fatal. It is incompatible with oxidisers, alkalis, and certain metals. Adrenaline stimulates the alpha- and beta-adrenergic systems, causing vasoconstriction, increased heart rate, and gastrointestinal relaxation.
The manufacturing process for adrenaline (also known as epinephrine) begins with incoming material quality control, where raw materials received from suppliers undergo visual inspection. The synthesis of adrenaline starts with the amino acid tyrosine, which is transformed through a series of chemical reactions to produce norepinephrine in the adrenal medulla. An enzyme called phenylethanolamine N-methyltransferase, found in the adrenal medulla, then catalyses the methylation of norepinephrine to produce epinephrine (adrenaline).
Afterwards, the adrenaline is subjected to an injection moulding process (Assembly 1), followed by visual inspection and electrical resistance measurement (Test 1). Next, welding and coating take place (Assembly 2), and the product undergoes cleaning. Visual inspection, leakage current measurement (Test 2), and further cleaning are performed before packaging and labelling. Bioburden testing (Test 3) ensures the product meets sterility requirements. Ethylene oxide sterilisation is then carried out, and a final visual inspection (Test 4) is conducted. The finished adrenaline products are packaged in outer containers and distributed to end users.

Adrenaline, also known as epinephrine, is a hormone and neurotransmitter used in medical emergencies. The procurement of adrenaline involves the following steps:
Step 1: Formation of 3,4-Dihydroxybenzaldehyde
Starting with catechol, it is formylated to produce 3,4-dihydroxybenzaldehyde.
Chemical reaction: C6H4(OH)2 + HCHO → C6H3(OH)2CHO
Step 2: Formation of 3,4-Dihydroxyphenylacetaldehyde
3,4-Dihydroxybenzaldehyde undergoes reduction to form 3,4-dihydroxyphenylacetaldehyde.
Chemical reaction: C6H3(OH)2CHO + H2 → C6H3(OH)2CH2CHO
Step 3: Formation of 3,4-Dihydroxyphenylacetone
3,4-Dihydroxyphenylacetaldehyde is further reduced to 3,4-dihydroxyphenylacetone.
Chemical reaction: C6H3(OH)2CH2CHO + H2 → C6H3(OH)2CH2CH2OH
Step 4: Formation of Adrenaline
Finally, 3,4-dihydroxyphenylacetone undergoes methylation and amination to form adrenaline.
Chemical reaction: C6H3(OH)2CH2CH2OH + CH3NH2 → C6H3(OH)2CH(OH)CH2NHCH3
In medical applications, adrenaline is used to treat severe allergic reactions (anaphylaxis), cardiac arrest, and low blood pressure associated with septic shock. It is also used to relieve respiratory distress due to bronchospasm and to prolong the action of local anaesthetics. Additionally, adrenaline is used in inhalers to improve symptoms of croup and in auto-injectors for emergency treatment of anaphylaxis. It is also used as a haemostatic agent and to treat conditions such as open-angle glaucoma and uterine contractions. The drivers of adrenaline's use include its ability to rapidly restore cardiac rhythm, increase blood pressure, and alleviate severe allergic reactions, making it a vital medication in emergency medical settings.
